Copyright Transfer Agreement and Disclosure Form
Download and print the PDF of the Transfer of Copyright Agreement, Conflict of Interest Acknowledgment, Certification of Coauthors, Exclusive Publication Statement, and Disclosure of Commercial Interest form. This form must be signed by all of the authors listed on the manuscript.
